Coverage Start and End Dates

Health Care, Dental, Vision Care, Life Insurance, Accidental Death & Dismemberment Plans

This information is for active plan members of UA Local 740’s Health & Welfare Plan.  If you are retired, disabled, or a plan member under a different plan, please refer to the Getting Started page for your plan administrator’s contact details.

Initial Coverage:

Coverage begins on the first day of the second month following the accumulation of three (3) months premium in your dollar bank account (currently $825).

Reinstating Coverage:

Coverage begins on the first day of the second month following the accumulation of two (2) months premium in your dollar bank account (currently $550).  Plan Members working as a permit worker or temporary foreign worker through the Union may be reinstated on the first day of the second month following the month you have accumulated three months of coverage (currently $825) in your Dollar Bank Account.

A dollar bank account is an account kept by the Plan Administrator for each Plan Member, which shows the value of hours reported monthly by your employer.  The employer will contribute to your dollar bank an amount per hour based on the rate negotiated in the collective agreement or special project agreement.

Note: Active Plan Members must be at work or available for work on the date benefit coverage begins. Should you not be working or available for work on the day your benefit coverage would ordinarily start, the benefit coverage for you and your dependents will be delayed until you return to work or are available for work.

Coverage Ends:

A Plan Member’s coverage will terminate on the earliest of the following dates:

  • The last day of any month in which the Plan Member has less than the required minimum in your Dollar Bank Account, provided the Member did not exercise the self-payment option;
  • For Members who are self-paying, the date that the maximum allowable number of self-payments has been made;
  • For Members who are self-paying, the date the Member ceases to make a self-payment;
  • The date the Plan Member is no longer a full-time resident of Canada;
  • The date the Plan Member is no longer covered by his/her Provincial health plan;
  • The date the Plan Member is incarcerated after committing a criminal offence for which he/she was found guilty;
  • The date the Plan Member enters military service (i.e. commences active duty in the armed forces of any country, state or international organization);
  • The date the Group Master Policy is cancelled or terminated;
  • The date of your death;
  • In addition to the above:
  • the date the Member retires for Accidental Death & Dismemberment and Dependent Life Insurance are not available to Retired Members for self-pay;

A Dependent’s coverage will terminate on the earliest of the following dates:

The date the Plan Member of whom he/she is a dependent ceases to be covered under the Plan;

The date the dependent ceases to be a dependent as defined in the group policy;

The date the dependent is no longer a full-time resident of Canada;

The date the dependent is no longer covered by the provincial health plan;

The date the dependent commences active duty in armed forces of any country, state or international organization;

The later of the following dates:

  • the date indicated on a written notice received from the Trust Fund;
  • the date this notice was received by the insurer.

You will receive a notice by mail advising you of the coverage effective date and coverage termination date.  It is important that you keep your mailing address current.  You can update your mailing address by logging on to your ‘myManion’ account or by contacting the Plan Administrator.

AIG Emergency Out of Province Medical Coverage Plan

Coverage for Emergency Out of Province Medical treatment is provided by AIG Insurance Company of Canada for eligible Plan Members under age 80.  This information is for active plan members of UA Local 740’s Health & Welfare Plan.  If you are retired, disabled, or a plan member under a different plan, please refer to the Getting Started page for your plan administrator’s contact details.

Initial Coverage:

Coverage begins on the first day of the second month following the accumulation of three (3) months premium in your dollar bank account (currently $825).

Coverage Ends: 

An Insured Person’s coverage ends on the earliest of:

  • The date the plan is terminated;
  • The premium due date if premiums are not paid when due by the Plan Member’s Plan Administrator;
  • The date you no longer satisfy the definition of an Insured Employee, or, for an eligible dependent, the date such dependent no longer satisfies the definition of Spouse or Dependent Child, as applicable;
  • The first day of the month following the date the Plan Member no longer belongs to an eligible class of members.
  • When a Plan Member turns age 80, this coverage ceases for such member as well as his/her eligible dependents. Coverage for a Spouse may terminate sooner if he or she attains age 80 before the Plan Member’s coverage terminates. Similarly, coverage for a dependent child will cease once he or she no longer satisfies the applicable criteria provided within the definition of Dependent Child.

Member Family Assistance Program

This program is provided by the RDTC/NLBTC Members’ Trust through Shepell.

Coverage Begins for you and your immediate family members on the day you become a member of UA Local 740.

Coverage Ends for you and your immediate family members on the day you become expelled from UA Local 740.

Substance Abuse and Rehabilitation Program
(UA Local 740 Members Only)

This program is provided by the RDTC/NLBTC Members’ Trust through Shepell for members only.  The parameters of this program will be communicated to the member upon contacting the plan administrator, Corey Parsons at 1-709-832-5116.  This is a confidential and voluntary support program with no direct cost to the member.
